POSITION SUMMARY
At Kyabra, our service teams use a strengths-based approach while working alongside people in our community to support them as they navigate the circumstances and systems that significantly impact their lives while helping them identify and achieve their personal goals.
The Case Management and Coordination (CMC) team is responsible for providing case management support to, and coordinating services for, community members requiring support with complex issues. The team operates on an integrated model that supports community members seeking assistance across any of the following areas:
- Disability Support Services (as a registered NDIS Provider for Support Coordination and Specialty Disability Accommodation)
- Housing Support Services
- Foster and Kinship Care Support Services
The Case Worker will ensure that the most appropriate and effective support services are provided to a range of individuals and families through case work that supports their aspirations and promotes meaningful change in their lives. Due to current organisational needs, the successful candidate will be expected provide both Disability Support Services (via the provision of Support Coordination to NDIS participants) and Housing Support Services to a caseload of community members.
Hours of Work
Applicants with the capacity to work full-time hours (76h 00m per fortnight) are preferred, however, applicants with part-time availability will be considered. Part-time applicants must be available to work a minimum of 60h 48m per fortnight.
KEY SELECTION CRITERIA
Qualifications & Training
- Tertiary qualification (bachelor's degree or higher) in Human Services, Social Work, and/or another relevant field.
Skills, Knowledge & Experience
- Demonstrated experience working in the community service sector in one or more of the following areas: intake and assessment, case management, disability support, housing support services, child protection etc.
- Demonstrated understanding of the Disability Services and Inclusion Act (2023), Residential Tenancies and Rooming Accommodation Act (2008), and other relevant human services legislation.
- Understanding of, and commitment to, the principles of social justice, as well as a demonstrated capacity to work in ways that focus on people’s strengths.
- Strong communication, interpersonal and organisational skills, including an ability to work autonomously or as part of a team, and adapt to various circumstances as necessary to support the organisation and/or community members.
History Checks & Clearances The successful applicant will be required to hold a positive/satisfactory result for the following:
- Blue Card (Working with Children Check, Queensland)
- NDIS Worker Screening
- LCS2 (Personal and Criminal History Check)
- Driver’s license (Queensland).
Desirable
- Demonstrated experience in providing Support Coordination services to NDIS participants.
- Demonstrated understanding of the issues/barriers facing people who are homeless or at risk of homelessness, living with a disability and/or navigating child protection systems (foster and kinship care), including ways to resolve or improve these issues.
- Knowledge of client management systems and the ability to rapidly learn new ones such as SRS, QHIP and/or other systems/databases.
- Existing community connections and referral networks, or the ability to rapidly acquire them.
