The Salvation Army Australia’s Homelessness Stream provides high quality, person-centred services to people at risk, and/or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services. Services are free, voluntary, accredited and delivered by professional and committed staff. About the role We are seeking a values-driven
Case Worker who will provide case management support in a community setting, supporting ADF veterans and veteran families to secure housing, experience improved social inclusion and achieve housing stability across Queensland. This support is achieved through person-led case management incorporating resilience, trauma informed and strengths-based practices whilst working effectively with other members of the team to provide a comprehensive service that reflects the vision, mission and values of The Salvation Army.
You will be successfully Establishing relationships with individuals that access the service that encourage self-determination and participation. Providing case management support to individuals in accordance with TSA case management models and standards of practice. Completing thorough assessments inclusive of needs, risks and vulnerabilities.
Providing ongoing assessment of the individual’s presenting issues, vulnerabilities and needs from initial intake to exit. Managing individual case planning in collaboration with the individual including a range of interventions that address the needs or goals of the individual, as well as barriers to achieving those. You will have Tertiary qualification in Community Services, Social
Work or related field. A degree level qualification is preferred. Relevant experience in a social service setting including in client assessment and case management QLD
Working with Children Check (position is located at a site that provides overnight accommodation to young people and/or provides support to families in a home setting)
Driver’s Licence First Aid Certificate This is a permanent position and can be located anywhere in Queensland.
Salary and conditions are in accordance with the Social, Community, Home Care and Disability Services Level 4 Award. What we offer The Salvos offer eligible employee's a well-balanced package of meaningful benefits including:
