Clinical Care Partner (Case Management) - AHPRA Registered

Are you a natural people-person who loves solving problems, coordinating support, and making a real difference? We’re looking for a proactive, compassionate, and forward-thinking Clinical Care Partner to join our Sydney Northern Beaches Team.

This isn’t your average case management role — with the rollout of the Support at Home Program, you’ll have the chance to lead with impact, empower clients, and help older Australians live life their way.

About the Role

As a Care Partner, you’ll be the go-to person for coordinating care that truly puts people first. You’ll work side by side with clients and families to navigate services like allied health, nursing, and home support.

Think of yourself as the connector, advocate, and coach — helping clients stay independent while living life on their own terms.

What You’ll Be Doing

- Manage and coordinate a caseload of Support at Home (SAH) & Private clients
- Conduct client assessments, reviews, and care planning
- Build meaningful relationships with clients, families and healthcare professionals
- Ensure clients receive services that meet their goals and changing needs
- Monitor budgets, services, compliance, and client outcomes
- Advocate for clients and provide exceptional customer service

About You You’ll shine in this role if you’re organised, empathetic,



and passionate about improving lives.

Must-haves:

- Experience in aged care case management (HCP/SAH or CHSP preferred)
- Background in community or aged care service coordination
- Excellent communication and planning skills
- Ability to work independently and as part of a team
- Current driver’s licence
- Able to obtain a Police Check
- Current practicing AHPRA Registration
