Vis-Ability Support Services Pty Ltd is seeking a compassionate and motivated Community Worker to join our team supporting people with disability and mental-health-related needs. We provide person-centred support, occupational therapy, support coordination and recovery coaching services across Sydney. Our aim is to empower clients to build independence, achieve their goals and actively engage in their communities.

Key Responsibilities

- Assess clients' needs and assist in developing appropriate support plans.
- Provide person-centred assistance that promotes independence and community participation.
- Support clients with daily living activities, social participation and access to community services.
- Liaise with families, carers, healthcare professionals and other service providers.
- Assist clients to access health, welfare, housing, employment and community resources.
- Monitor client progress and maintain accurate case notes and service records.
- Follow NDIS requirements, workplace policies and the NDIS Code of Conduct.
- Promote clients' rights, dignity, choice and self-determination.

Essential Requirements

- Relevant qualification in Community Services, Disability, Mental Health, Welfare or a related field.
- Experience supporting people with disability and/or mental health-related support needs.
- Strong communication,



organisational and problem-solving skills.
- Ability to work independently and collaboratively within a professional team.
- Current NDIS Worker Screening Check and Working with Children Check (where applicable).
- Current Australian Driver Licence and reliable transport.
- Commitment to person-centred, culturally safe and inclusive practice.

Desirable

- Experience working within the NDIS.
- Current First Aid and CPR certification.
- Knowledge of Support Coordination, Recovery Coaching or Occupational Therapy services.

Pay: $80,000.00 – $100,000.00 per year
