Vitality Nursing and Disability Services is a community care provider supporting NDIS participants in their homes. We pride ourselves on delivering skilled services through experienced nurses with a friendly, open, talkative and personable manner.
Responsibilities include conducting comprehensive continence assessments, developing clinical reports, recommending interventions, and creating management plans to improve clients' continence outcomes.
We are currently seeking a Continence Clinical Nurse to join our organisation on a casual basis for approximately 5-10 hours per week, with the opportunity to increase hours based on your availability.
This is a community-based role located in the Perth Metro area. The position involves a combination of home visits to complete in-person assessments and remote work (at home) to complete reports. We have a Continence Assessment template which we will use for all Reports. The role also involves ongoing case management and documentation.
Previous learned knowledge and experience in Urology, Continence, Stomas, Catheters is preferred.
Casual contract pay rate $61.00/hr plus superannuation%
The role involves but is not limited to:
· Collection of detailed information during an assessment of continence and report writing
· Medication Administration and Complex Clinical Care ie. bowel care/suppositories, IDC & SPC catheter care, stoma care and more
· Providing continence education to support workers
· Providing direct care to clients with incontinence or those at risk of developing incontinence
· Referring as appropriate to other health professionals
· Undertake case management for a portfolio of clients
Desirable requirements:
· Completion of a Post graduate certificate/diploma in Continence Assessment, Management and Promotion
· Strong communication and interpersonal skills.
· Proficient use of Microsoft Word, Excel.
· Ability to work in a multidisciplinary team
· Current AHPRA registration as a Registered Nurse
· Current Driver’s License
· Current National Police Check
· Current Working with Children Check
· Required to apply for an NDIS Worker Screening Check – valid for 5 years
· Ability to work unsupervised in a clinical and remote setting
