This position qualifies for an annual Recruitment Incentive Package up to the value of $10,000 (conditions apply and may be pro-rata’d to your contracted or ordinary working hours, these payments are paid on a fortnightly basis) under the NSW Health Rural Health Workforce Incentive Scheme.

This scheme recognises the role key health workers play in enabling the delivery of rural and remote health services, and aims to provide incentives over and above award entitlements, to attract and retain staff in these areas.

What You'll Be Doing The Senior Allied Health Clinician – Disability and Complex Care provides advanced clinical assessment, intervention, consultation, care coordination and service development for people with disability and complex health needs across Far West Local Health District.

- Support the Hospital Social Work team and multidisciplinary teams in the management of complex patients, including discharge planning, guardianship matters, accommodation challenges, psychosocial complexity, NDIS interface issues and transitions of care.
- Provide expert advice, consultation and capacity building to clinicians regarding the assessment and management of people with disability accessing health services.
- Facilitate coordination between NSW Health services, disability providers and community agencies to support safe admissions, discharge planning and transitions of care for people with disability and complex needs. Develop and maintain strong stakeholder relationships to improve integration between health, disability and community services and promote positive patient outcomes.
- Identify and respond to barriers impacting access to health services for people with disability and work collaboratively with stakeholders to develop and implement solutions.

How To Apply To be considered for this position,



please ensure you address the selection criteria in the application as thoroughly as possible:

- Degree qualification in Occupational Therapy, Physiotherapy, Social Work with current registration with AHPRA and/or AASW where applicable, or eligibility for membership of the relevant professional association.
- Extensive clinical experience working with people with disability and/or complex health needs within health, community or disability settings.
- Demonstrated experience managing complex care coordination, discharge planning and multidisciplinary care pathways.
- Demonstrated ability to provide specialist consultation, education and support to clinicians and multidisciplinary teams.
- Demonstrated understanding of disability service systems, including the NDIS and broader health-disability interface.
- Demonstrated ability to work autonomously whilst contributing effectively within a multidisciplinary and Social Work-led team setting.
- Demonstrated experience leading quality improvement, service development, project work or redesign activities.
- Highly developed communication, negotiation, advocacy and stakeholder engagement skills.
