As a Disability Support Worker, you will provide individualised support based on each participant's needs, preferences, goals and support plans. You will support NDIS participants across community access, day programs and individual support settings.
Key responsibilities
- Supporting participants with community access and social activities
- Providing 1:1 and group-based supports
- Assisting with daily living activities and personal care where required
- Supporting participants to work towards their individual NDIS goals
- Following individual support plans, Behaviour Support Plans and risk assessments
- Supporting participants who may display behaviours of concern
- Providing protected transport to appointments, activities and community outings
- Encouraging independence, choice and decision-making
- Completing accurate progress notes, incident reports and other required documentation
- Maintaining professional boundaries and participant confidentiality
