About the Role - URGENT

We are currently seeking a compassionate, experienced, and reliable Family Support Worker to support a participant with complex physical, neurological, and mental health needs .The role involves assessing clients' needs, developing and implementing support plans, monitoring progress, and connecting clients with appropriate community resources and services.

Family Support

Workers work closely with families, community organisations, government agencies, and other stakeholders to promote wellbeing, independence, and positive outcomes. They also maintain accurate records, preparereports and funding submissions, and advocate for clients to ensure they receive the support and opportunities they need to improve theirquality of life.

Key Responsibilities

- Conducting client interviews to identify and evaluate the nature, scope, and impact of their challenges and support needs.
- Monitoring client progress and maintaining accurate records and reports on outcomes and interventions.
- Assessing community needs and available resources related to health, welfare, housing, employment, education, and training services.
- Building and maintaining relationships with community organisations,



welfare agencies, government departments, and private sector stakeholders to enhance community support and resource awareness.
- Providing guidance, support, education, and care to families, children, and people with disabilities in a range of community and residential care settings.
- Assisting people in addressing social, emotional, behavioural, and financial challenges through appropriate support and intervention strategies.
- Preparing funding applications, grant submissions, and comprehensive reports for government agencies and other relevant stakeholders.

Requirements

- Strong Assessment and Case Management Skills
- Knowledge of Community Services and Support Networks
- Excellent Communication and Relationship-Building Skills
- Experience Supporting Vulnerable Individuals and Families
- Robust Documentation and Reporting Skills
- High-level empathy, patience, and compassion
- Valid NDIS Worker Screening Check
- Driver’s license and reliable transport - must own a suitable vehicle.
