Melbourne Community Home Care Services is a new Home Care provider seeking an experienced Case Manager to join us on a part-time basis, working 3 days per week.
This is an opportunity for an experienced Home Care professional to join us from the beginning and play an important role in establishing and growing our Home Care services.
About the Role
You will work closely with the Director to establish strong case management processes and provide high-quality, person-centred services as our client base grows.
Your responsibilities will include
- Managing Home Care clients from intake and onboarding through to ongoing service delivery
- Conducting assessments and home visits
- Developing and reviewing individual care plans
- Preparing and monitoring client budgets
- Assisting clients to navigate My Aged Care
- Coordinating support workers and other services
- Responding to recent client enquiries and referrals
- Meeting prospective clients and assisting with service commencement
- Liaising with clients, families, GPs, allied health professionals and other service providers
- Monitoring client needs, risks and service quality
- Maintaining accurate client records, case notes and documentation
- Supporting compliance with the Aged Care Quality Standards
- Helping establish effective case management systems and processes
- Building relationships with healthcare professionals, community organisations and referral networks
- Supporting business development and growth of our client base
About You We are particularly interested in someone who understands Home Care and is comfortable joining a new organisation during its growth stage.
Ideally, you will have:
- 2+ years' experience in Home Care case management or care coordination
- Previous experience managing Home Care clients
- Experience preparing and managing client budgets
- Strong knowledge of My Aged Care
- Strong understanding of the Aged Care Quality Standards
- Experience developing care plans and coordinating services
- Excellent communication and relationship-building skills
- Strong organisational and time-management skills
- Confidence working independently
- Current National Police Check
- Current First Aid and CPR
- Valid driver's licence and reliable vehicle
- Existing community and industry relationships are highly regarded
- NDIS knowledge or experience is desirable
