Department of Health Tasmania is seeking a Clinical Nurse Consultant for the Rapid Access Service pilot in Launceston and Burnie.
The role focuses on timely assessment, expert care recommendations, and follow-up for older Tasmanians with BPSD in residential aged care homes.You will lead a multidisciplinary team, coordinate referrals, and ensure data integrity while aiming to keep residents at home where possible.
