Bendigo Health is seeking a Project Officer to support the Transition Care program in the Loddon region.
The role focuses on reviewing referral pathways, occupancy and barriers, and developing a versatile service model to maximise program use and equitable access.
You will oversee data collection, KPI reviews, and produce reports with regular stakeholder engagement.
