3 days a week across Monday to Friday | No Shift Work | On-site in Residential Aged Care Homes

This is not a work from home role, and it isn't a graduate position.
We're looking for an experienced Registered or Enrolled Nurse to work3 days a week across the Atherton Tablelands in Far North Queensland.
You need to be a confident clinician who can assess situations, recognise risk, prioritise competing demands and make clinical decisions independently.
You'll coordinate care, work closely with GPs and Specialists and make sure residents receive the right care at the right time.

About Access Aged Care

Through Access Aged Care, we partner with Residential Aged Care Homes across Australia to deliver co-managed primary care to older Australians.

What the Role Looks Like
As an Access Aged Care Nurse, you'll workon-site within Residential Aged Care Homes across the Atherton Tablelands.
You won't replace the nursing team within the home. You'll work alongside them, bringing another layer of clinical coordination and helping ensure residents don't fall through the gaps.
One hour you might be assessing a resident ahead of a GP round. The next you could be coordinating Specialist follow-up, reviewing a clinical concern with the care team, completing an assessment or making sure an important referral keeps moving.
You'll need to think critically, adapt quickly and be comfortable making decisions without someone looking over your shoulder.
Your responsibilities will include:
Coordinating GP and Specialist rounds, both face-to-face and via telehealth.
Completing comprehensive clinical assessments.
Identifying clinical concerns early and escalating appropriately.
Coordinating referrals, investigations and follow-up care.
Ensuring agreed clinical actions are followed through.
Communicating with residents, families, GPs, Specialists and Residential Aged Care Home teams.
Maintaining accurate and timely clinical documentation.
Using multiple clinical systems, telehealth platforms and digital tools.
Building strong relationships with Residential Aged Care Home teams.
Supporting high quality resident outcomes through effective clinical coordination.

This role won't suit you if you:
Are a graduate or still developing the clinical experience required to make confident, independent decisions.
Are looking for a traditional ward or bedside nursing position.
Prefer highly structured environments where your day and tasks are prescribed for you.
Don't enjoy using a variety of technology as part of your everyday clinical work.
We'd rather be upfront. This role requires an experienced nurse who can work with significant autonomy. It is not designed as an entry point for graduate or early career nurses or anyone who isn't confident with technology. We're a digital focused company.
What You'll Bring
Current AHPRA registration as a Registered Nurse or Enrolled Nurse.
Minimum 2 years' nursing experience following AHPRA registration, with demonstrated experience working autonomously.
Aged care experience or experience in another relevant clinical environment.
Strong clinical reasoning and the ability to recognise and respond to risk.
Confidence making decisions independently and knowing when to escalate.
Strong organisation and prioritisation skills.
Clear, confident communication with multiple stakeholders.
Confidence using technology and learning recent systems.
A current driver's licence and reliable vehicle.
Flexibility to travel between RACHs across the Atherton Tablelands as operational needs change.
