Townsville Hospital and Health Service in Townsville, NC QLD, is seeking a Nurse Navigator in Complex Chronic Rheumatology. This permanent part time position (56 hours per fortnight) supports patients by coordinating care journeys and building strong links with primary care providers.

The role focuses on proactive, patient-centred support to improve health outcomes, with a nursing qualification (Bachelor of Nursing) required and post-graduate qualifications highly desirable.
