You will manage a caseload of clients receiving government-funded aged care services, conduct home visits, and coordinate support to meet client goals.
You will work with families, clinical teams and external providers, monitor wellbeing, and participate in an after-hours on-call roster.
Essential qualifications include aged care experience and knowledge of quality standards.
