ATSICHS Brisbane is seeking a Care Coordinator Nurse to support patients with Chronic Illness and coordinate services to improve health outcomes for Aboriginal and Torres Strait Islander peoples. You will work closely with clients and a multidisciplinary team in a culturally protected setting.

The role offers permanent full-time employment with a starting salary of AUD 78,447 plus 12% super, with additional net salary packaging and fringe advantages.
