CatholicCare Central Queensland – Rockhampton QLD is seeking a Dementia Together Case Manager to cover maternity leave through May 2027. The role delivers assessments, guidance, referrals and tailored care plans to help people living with dementia and their carers maintain quality of life at home.

You’ll coordinate services, support families, and promote education sessions within a team-oriented team, supported by ongoing training and development prospects.
