Western Health's Mental Health and Wellbeing Service is seeking a Community Mental Health Case Management clinician to deliver recovery-focused, person-centred care in a energetic community setting.

As an RPN3, you will work within a multidisciplinary team, maintain documentation, assess risks, and coordinate services to support consumer safety and wellbeing. Victorian driver licence preferred. Based in Melbourne, VIC.
