Hunter Current England Local Health District is seeking a Permanent Full Time Registered Nurse to support Aboriginal health initiatives in Adamstown or Raymond Terrace. The role is part of NSW Health's Integrated Care Coordination and Prevention Program, delivering culturally protected, person-centred care.

You will conduct health assessments, provide education and self-management support, and coordinate access to community services while contributing to chronic disease prevention and management.
