ForHealth Group is seeking a Registered Nurse or Enrolled Nurse to join Kedron Medical Centre on a part-time basis, focusing on Chronic Disease Management and preventative care in collaboration with GPs and patients.

You will perform health assessments, coordinate GPMPs/TCAs, conduct screenings, and support patients with education and ongoing care, with a supportive team and opportunities for career development.
