Are you an experienced Registered Nurse with a strong background in aged care or community nursing?
Mt Gravatt Community Centre Inc. (MGCCI) is seeking an experienced Community RN Care Partner to join our Aged Care team.
This is a genuine community nursing role combining direct client care with clinical risk management, clinical guidance and clinical governance. You will work closely with clients and their families while also contributing to the quality, safety and continuous improvement of our aged care services.
We are looking for an RN who brings strong clinical judgement, excellent communication and organisational skills, sound knowledge of aged care legislation, regulations and standards, and the ability to work both autonomously and as part of a multidisciplinary team.
About the role
As an RN Care Partner, you will provide clinical assessment, care planning, nursing support and care coordination to older people in their homes and community.
You will work closely with clients, families, Support Workers, Care Partners, GPs, allied health professionals and other service providers to ensure care is safe, coordinated, person-centred and responsive to changing needs.
The role also has a strong clinical governance component. You will contribute your nursing expertise to clinical risk management, quality reviews, clinical audits, incident and complaint reviews, care planning and continuous improvement.
You will be expected to use your professional nursing judgement to identify concerns, determine appropriate action and escalate matters when required.
Key responsibilities
- Conduct clinical and health assessments and identify changes in client health, function, wellbeing and risk
- Develop, implement and review person-centred care plans
- Provide community nursing support, education and advice within your scope of practice
- Monitor changes in client circumstances and coordinate appropriate review, referral or escalation
- Work collaboratively with GPs, allied health professionals, hospitals and other health and community services
- Provide clinical guidance to Support Workers and Care Partners
- Identify and respond to clinical risks and contribute to incident, complaint and safeguarding reviews where there is a clinical component
- Undertake clinical audits and quality checks of care planning, clinical documentation and service delivery
- Contribute to clinical governance, continuous improvement and the review of policies, procedures and practice guidance
- Support the aged care team to maintain safe, evidence-based and person-centred practice
- Maintain accurate, timely and professional clinical documentation
About you
This role requires an experienced RN with a strong understanding of aged care and community based service delivery.
You will have
- Current AHPRA registration as a Registered Nurse (Division 1)
- A minimum of two years' demonstrated experience in aged care, community nursing or a related health setting, with experience supporting older people
- Sound knowledge of aged care legislation, regulations, the Aged Care Quality Standards and relevant quality and compliance requirements
- Robust clinical judgement and the ability to independently assess risk, make appropriate clinical decisions and know when to escalate
- Excellent written and verbal communication skills, with the ability to communicate effectively with clients, families, staff and health professionals
- Strong organisational skills, with the ability to manage competing priorities, maintain accurate records and follow through on actions
- The ability to work autonomously while contributing effectively as part of a multidisciplinary team
- A person-centred and culturally responsive approach to working with older people and their families
- Robust computer literacy and confidence using electronic client management and record-keeping systems
- A current Queensland driver's licence and ability to travel to clients in the community
Experience contributing to clinical governance, clinical audits, quality improvement or service reviews will be highly regarded.
