Care Partners Australia is a specialist provider of multicultural support services providing exceptional care and support across NSW and VIC.
We are an accredited and registered NDIS service provider offering short and medium-term accommodation and assistance for people with disabilities including adults, children, and teenagers in Sydney. We also offer a range of Specialised Disability Accommodation properties across NSW in regional and metropolitan areas.

Our staff consists of trained and compassionate members from a diverse range of communities. We are seeking a Residential Care Officer to join our team. The candidate will primarily be responsible for providing care and support to residents within residential facilities.
Responsibilities
Provide care, support and supervision to residents in accordance with their individual support plans, care plans and assessed needs.
Lead and supervise residential care staff to ensure residents receive safe, respectful and person centred care.
Coordinate the day-to-day delivery of care and support within the residential facility, including allocating duties, monitoring shift activities and ensuring continuity of care across the staff team.
Assess and regularly review residents’ care and support needs, taking into consideration their age, developmental stage, disability, cultural background, behaviours of concern and individual circumstances.
Guide staff in implementing residents’ care plans, behaviour support plans, health management plans, risk assessments and daily routines.
Implement individual support strategies that assist residents to develop independent living skills, participate in community activities and progress towards the goal documented in their individual support plans.
Provide leadership, direction, mentoring and on-the-job guidance to residential care staff, conduct staff handovers, address practice issues within the scope of the position,
and escalate material performance concerns to management.
Monitor residential care practices within the facility to ensure staff follow residents’ support plans, organisational procedures, workplace health and safety requirements, applicable NDIS Practice Standards and authorised restrictive-practice protocols and report identified non compliance to management.
Monitor residents’ physical, emotional and mental wellbeing and ensure that any changes, concerns or emerging risks are documented, reported and escalated appropriately.
Review staff progress notes, incident reports, medication records, daily logs and other documentation to ensure records are accurate, complete and submitted within the required timeframes.
Respond to incidents, emergencies and behaviours of concern, providing direction to staff and ensuring appropriate reporting, escalation and follow-up actions are completed.
Provide residents and their families with information, emotional support, practical guidance and assistance in understanding and accessing the appropriate support services, together with regular updates regarding the resident’s care, progress and changing support needs.
Build and maintain professional relationships with residents, families, guardians, support coordinators, community organisations, welfare organisations and other relevant stakeholders.
Collaborate with healthcare and allied-health professionals involved in residents’ care, including general practitioners, psychologists, behaviour support practitioners, physiotherapists, occupational therapists and other service providers.
Coordinate residents’ appointments,
health reviews and referrals and ensure that recommendations from health and allied-health professionals are communicated to staff and implemented appropriately. Refer residents to external agencies and specialist services where additional assistance needed.
Assist residents in accessing appropriate education, employment, training, healthcare, welfare, recreational and community-based programs and services, including identifying suitable community resources and facilitating referrals or connections where required.
Plan, coordinate and facilitate recreational, therapeutic and community-participation activities that promote social inclusion, cognitive stimulation, independence and physical wellbeing.
Monitor and evaluate the effectiveness of support services in meeting residents’ goals and improving their overall wellbeing.
Seek and respond to feedback from residents, families, staff members and relevant professionals to support continuous improvement in service delivery.
Identify and report risks relating to residents, employees, the residential environment and service delivery and implement appropriate risk-control measures.
Support the induction and training of recent residential care staff and ensure employees understand residents’ needs, routines, risks and support requirements before working independently.
Conduct regular team meetings, shift handovers and case discussions to communicate important information and promote consistent support practices.
Ensure residents’ privacy, dignity, rights, personal choices and cultural needs are respected and protected at all times.
· Report operational concerns, staffing matters, incidents and changes in residents’ needs to senior management and contribute to the development of appropriate solutions.
Successful applicants must have
· Relevant qualification or work experience in lieu
· At least 12 months of work experience in similar role
· Ability to work well in a team
· Excellent communication skills

Pay: $100,098.94 – $125,107.26 per year
