You will manage a diverse caseload in a fast-paced, multidisciplinary environment and coordinate recovery-focused support.
You will work with consumers, families and partners to deliver person-centred care, crisis intervention, discharge planning and ongoing clinical review, contributing to better outcomes for diverse communities.
