Central Highlands Rural Health Service is seeking a compassionate Care Partner to join our Care at Home team in Ballarat, Victoria. You will coordinate services, support goal achievement, and deliver person-centred care so clients can live safely and independently at home.

There are multiple permanent, part time positions available with hours negotiated with successful candidates. We value teamwork, inclusivity and high-quality community care across our rural campuses.
