Hazel Home Care on the Sunshine Coast is seeking a Care Manager with a Nursing Qualification (RN) to lead in-home support services. This role offers hybrid work, blending from home with a weekly visit to the Sippy Downs office, and aims to deliver bespoke care to a defined client portfolio.

You will develop and tailor Care Plans, coordinate with rostering and suppliers, and monitor client budgets to ensure sustainable care.
