Two excellent opportunities are available for registrars to join a newly established Hospital in the Home medical team at a major regional health service in Victoria.

n

These standalone 12-month positions are suitable for doctors at BPT2 or BPT3 level who wish to develop their skills in acute and ambulatory medicine while providing hospital-level care to patients in their own homes.

n

The successful registrars will receive dedicated consultant support, structured supervision and a personalised professional-development plan aligned with relevant Royal Australasian College of Physicians training requirements.

n

The Position

n

The Hospital in the Home service manages a diverse range of acute medical and specialty patients who can safely receive hospital-level assessment, treatment and monitoring outside a conventional inpatient ward. Care is delivered through:

n

- A dedicated Hospital in the Home clinic
n
- Telehealth consultations
n
- Visits to patients' homes
n
- Collaboration with inpatient and specialty teams
n
- Coordination with nursing and allied health services
n

n

The registrar will provide day-to-day clinical management of HITH patients while working closely with supervising consultants, nursing staff, junior doctors and referring clinical teams.

n

Key Responsibilities

n

The successful applicant will:

n

n
- Assess and manage patients admitted to the HITH service
n
- Develop and review individualised treatment plans
n
- Monitor patients receiving acute treatment at home
n
- Conduct clinical reviews in the HITH clinic, by telehealth and in patients' homes
n
- Identify clinical deterioration and arrange escalation or hospital admission when required
n




- Coordinate investigations, medications and follow-up care
n
- Liaise with inpatient, emergency and specialty medical teams
n
- Work collaboratively with nursing, allied health and community providers
n
- Maintain clear and accurate clinical documentation
n
- Support safe admission to and discharge from the HITH service
n
- Supervise and mentor junior medical staff
n
- Participate in clinical governance, education and quality-improvement activities
n

Training and Professional Development

n

A professional-development plan will be developed with the Medical Director and tailored to the registrar's experience, learning objectives and physician-training requirements. The appointment offers:

Essential Requirements

n

Applicants should:

n

n
- Hold a recognised medical degree
n
- Hold current or eligible medical registration with the Australian Health Practitioner Regulation Agency
n




- Have postgraduate experience appropriate for appointment at registrar level
n
- Be undertaking or eligible to undertake Basic Physician Training at BPT2 or BPT3 level
n
- Possess sound acute medical assessment and management skills
n
- Demonstrate effective communication, organisation and teamwork
n
- Be capable of working across clinic, telehealth and home‐based settings
n
- Demonstrate an interest in ambulatory care and cutting-edge models of healthcare delivery
n
- Meet all occupational health and immunisation requirements, including the 2026 influenza vaccination
n

n

A current Australian driver's licence may be required because the position involves providing care in patients' homes.
