The Cairns and Hinterland Hospital and Health Service is seeking an experienced Occupational Therapist (Case Manager) to join the Transition Care Team in Cairns, QLD. You will deliver occupational therapy and case management as part of a multidisciplinary team to support community clients post-discharge.

You will work with diverse clients across Far North Queensland, providing clinical guidance to service managers and stakeholders while maintaining a adaptable, patient-centered approach.
