Alfred Health in Melbourne is seeking a Diabetes Clinical Nurse Consultant to lead diabetes education and clinical management for adults with type 2 diabetes across Caulfield Community Health Service and the DCCC.
The role requires collaboration with a multidisciplinary team, supporting insulin titration, CGM use, and outreach education at Access Health. Part time (64 hours per fortnight) and based at Caulfield Hospital.
