We are looking for an experiencedSupport Coordinatorto join our team in Warragul on a15-month fixed-term contract, providing maternity leave cover.
This is ahybrid role, offering a mix of office-based and work-from-home arrangements while supporting participants across the Gippsland region.
About the role
As a Support Coordinator, you will work closely with NDIS participants to help them understand and implement their plans, connect with suitable services and supports, and build their confidence to navigate the NDIS.
Your responsibilities will include:
Managing a caseload of NDIS participants
Supporting participants to understand and implement their NDIS plans
Connecting participants with suitable providers and community supports
Building strong relationships with participants, families, providers and other stakeholders
Monitoring plan utilisation and participant outcomess>
Assisting with plan reviews and gathering supporting information
Maintaining accurate case notes, records and documentation




Working collaboratively with internal and external stakeholders
About you
We are looking for someone who:
Has experience inSupport Coordinationor a similar NDIS role
Has a good understanding of theNDIS and participant plans
Is confident managing their own caseload and priorities
Has solid communication and relationship-building skills
Is organised and comfortable working independently
Has good written documentation and computer skills
Takes a practical and participant-focused approach to their work
