As assessment and transition coordinator, you will help people prepare for life after prison.

Key responsibilities

coordinate risk and needs assessments, parole applications and pre-release services.
n
support case management reviews and help track each person's progress.
n
coordinate programs, referrals and services that support people before and after release.
n
work with prisoners, prison staff and service providers to support transition plans.
