We are seeking a motivated and compassionate
Community Physiotherapist
to join our growing team. This is an excellent opportunity for a physiotherapist who enjoys working independently in the community while remaining part of a supportive and experienced team.
About the Role
You will provide in-home physiotherapy services to clients receiving support through the
Support at Home Program
and to
NDIS participants
, helping clients maintain or improve their mobility, independence, safety and overall quality of life.
You will be based at our
Mount Waverley office
and will travel to clients throughout metropolitan Melbourne and the Bass Coast region.
This is a
full-time position
.
$80,000 – $100,000 per annum + superannuation
, based on qualifications and experience.
Key Responsibilities
Your responsibilities will include:
Conduct physiotherapy assessments, including mobility, strength, balance, gait, pain, falls risk, post-hospital needs and assistive technology.
Develop and deliver individualised, evidence-based treatment plans aligned with each client's goals, needs and available funding.
Provide interventions such as strength and balance training, falls prevention, mobility and gait retraining, pain and chronic condition management, neurological rehabilitation, manual therapy and manual handling support.
Recommend assistive technology, mobility aids and home equipment where appropriate.
Monitor client progress and adjust treatment plans as required.
Prepare accurate clinical documentation and reports, including Functional Capacity Assessments and supporting documentation where required.
Work collaboratively with clients, families, Care Partners, nurses, support workers and other health professionals.




Participate in care planning and reviews and provide relevant education and advice.
Deliver services in accordance with Support at Home and NDIS requirements and applicable quality standards.
Skills and Experience
To be successful in this role, you will have:
A Bachelor's or Master's degree in Physiotherapy
Current registration with
AHPRA
A minimum of
1 year of physiotherapy experience
Strong clinical assessment, treatment planning, documentation and report writing skills
Ability to work autonomously and effectively manage a community-based caseload
Excellent communication, interpersonal and organisational skills
A client-centred approach with a robust commitment to promoting independence and quality of life
Highly Desirable
Experience in community physiotherapy, aged care, disability or private practice.
Knowledge of Support at Home and/or the NDIS.
Experience in falls prevention, neurological rehabilitation, manual handling, assistive technology or equipment prescription.
Other Requirements
The successful applicant will be required to have:
Current Victorian Driver Licence
Reliable vehicle with appropriate insurance
Current Police Check or NDIS Worker Screening Check
Current First Aid Certificate
Current CPR Certificate
