Level 2 Support Coordinator
to join our team, supporting participants across the NDIS, Child Protection, and Forensic Disability sectors.
This role requires a proactive professional who can work both collaboratively within a team and independently to deliver high-quality outcomes.

Level 2 Support Coordinator
to join our team, supporting participants across the NDIS, Child Protection, and Forensic Disability sectors.
This role requires a proactive professional who can work both collaboratively within a team and independently to deliver high-quality outcomes.

Key Responsibilities
Manage and support a caseload of approximately 20 participants
Develop and implement effective case plans
Assist participants and their families to understand and utilise their NDIS plans




Facilitate care team meetings with participants and key stakeholders
Build participant capacity and promote independence
Monitor plan budgets and outcomes to ensure effective utilisation of funding
Develop and maintain professional relationships while upholding appropriate boundaries
Liaise with mainstream and community services (e.g. Housing, Education, Justice, Health) to ensure obligations are met
Maintain accurate and compliant documentation, including case notes, reports, and billing records
Ensure compliance with relevant legislation, standards, and internal frameworks
Skills and Experience
Demonstrated experience working with people with disability
Background in community services, disability, mental health, or related sectors
Strong understanding of NDIS Practice Standards, Work Health and Safety, and Equal Opportunity principles
Ability to problem-solve, show initiative, and achieve positive participant outcomes
Knowledge and understanding of Aboriginal and Torres Strait Islander cultures (highly regarded)
Strong knowledge of local disability, mainstream, and community services, with proven networking ability
Experience within the Justice system (desirable but not essential)
Excellent organisational and time management skills




High-level verbal and written communication skills
Experience in person-centred planning and service delivery
Sound computer literacy, including report writing and documentation
Knowledge of the NDIS and relevant legislation, including the NDIS Act **** and Disability Act ****
Qualifications
Certificate IV in Disability, Community Services, or a related field (highly regarded but not essential)
Experience within the NDIS and disability sector (preferred)
Essential Requirements
Current Driver's Licence
Valid Working with Children Check (Employee)
Current National Criminal History Check (within the last 6 months)
NDIS Worker Screening Check
Evidence of Australian working rights
Current First Aid and CPR certification
NDIS Orientation Certificate
Evidence of COVID-19 vaccination (including booster) or valid medical exemption
