Key responsibilities

As Assessment and Transition Co-ordinator, you will support people through their prison journey.
n
support the Corrections Victoria Reintegration Pathway and assess rehabilitation and reintegration needs.
n
develop transition plans and parole applications, with transparent plans for release.
n
coordinate pre-release services and work with service providers to support people leaving prison.
n
