About the Role
We're looking for an experienced Service Coordinator with a strong background in NDIS service delivery and case management.
You'll coordinate services for a medium-sized, manageable group of participants, working closely with families, representatives, support coordinators, support workers, and external providers to deliver person-centred, high-quality supports.
This non-clinical role focuses on coordinating day-to-day services, maintaining strong stakeholder relationships, supporting frontline staff, and ensuring services meet participant goals and NDIS requirements.

Key Responsibilities
Build and maintain productive relationships with participants, families, representatives, support coordinators, and external providers
Coordinate participant services from onboarding through to ongoing review and service transition
Develop, review, and update support plans, service agreements, risk assessments, and progress reports
Ensure supports align with participant goals, preferences, funding, and NDIS plans
Monitor service quality, participant satisfaction, goal progress, and changing support needs
Conduct regular participant reviews, stakeholder meetings, and service check-ins
Work closely with rostering and operational teams to ensure consistent service delivery
Provide guidance and day-to-day support to disability support workers
Respond to participant concerns, incidents, service issues, and changes in circumstances
Maintain accurate case notes, participant records, and compliance documentation
Support incident investigations, corrective actions, and required reporting
Ensure services comply with NDIS Practice Standards and organisational policies
Identify risks, service gaps, and opportunities for continuous improvement
Your Experience and Qualifications
Strong service coordination and case management experience within the NDIS, disability, or community services sector
Demonstrated understanding of NDIS participant plans, funded supports, service agreements, and provider responsibilities
Sound knowledge of NDIS Practice Standards and Quality Indicators
Understanding of NDIS Commission requirements, including incident management, reportable incidents, complaints, and safeguarding
Experience coordinating supports for people with complex needs, preferably within SIL, accommodation, or community settings
Proven ability to coordinate multiple stakeholders and manage differing priorities
Experience developing participant support plans, risk assessments, and service documentation
Robust communication, organisation, problem-solving, and conflict-resolution skills
Ability to work independently while contributing effectively to a team
Experience supervising or supporting disability support workers
Relevant qualification in disability, community services, case management, or a related discipline is desirable
Valid SA driver's licence, First Aid certificate, NDIS Worker Screening Check, and required clearances
