The Justice Health and Forensic Mental Health Network invites applications for an Integrated Care Coordinator (HM1), TFT up to March ****, located in the Sydney Metro/MRRC. The role focuses on coordinating care for adults with chronic or complex health conditions across the patient journey from reception, through custody, to community settings.
