In this role, you will provide protected, evidence-based nursing care and support optimal health outcomes for residents and their families across Dhelkaya Health services.
Key responsibilities
Undertaking resident assessments and reporting clinical findings or changes to the Registered Nurse.
Contributing to the development and implementation of person-centred care plans.
Delivering direct care in line with individual resident needs, goals, and preferences.
Monitoring and evaluating care outcomes in collaboration with the Registered Nurse and care team.
