, have worked with NDIS participants through
hospital discharge and transition planning
, and have genuine relationships with Hospital Liaison Officers, Social Workers, discharge teams and other referral partners, I'd love to speak with you.
This could be a great next step for an experienced Social Worker or NDIS/health professional who loves building relationships and wants to move into a more commercially focused role.
