Key responsibilities

As an expert clinician, you will provide specialist leadership and work autonomously to deliver high-quality, patient-centred care across hospital and community settings.

You will play a vital role in bridging the gap between acute and community services, supporting care planning, rapid response pathways, and hospital avoidance initiatives.

Working closely with multidisciplinary teams, you will help drive creative programs and service improvements that enhance patient outcomes and the delivery of chronic liver disease care.

What you need
n
n
Effective leadership skills including highly developed skills in communication, problem solving, conflict resolution and negotiation skills.
n
Registered Nurse and or Midwife with at least 3 years post registration experience.
n
Demonstrated experience and competence in Chronic Disease Management.
