This part-time Social Worker - Community role at Hospital in Your Home in Brisbane, Queensland offers an exciting opportunity to make a meaningful difference in the lives of patients and their families. As a Social Worker, you will be responsible for providing compassionate and comprehensive social services to support patients and their loved ones throughout their healthcare journey.

What you'll be doing

n

Conducting comprehensive assessments to identify the social, emotional, and practical needs of patients and their families

n

Developing and implementing personalized care plans to address the unique needs of each individual

n

Providing counselling and emotional support to help patients and families cope with illness, disability, and end-of-life concerns

n

Collaborating with the multidisciplinary healthcare team to coordinate and facilitate access to community resources and services

n

Advocating for patient rights and ensuring they receive the appropriate level of care and support

n

Maintaining accurate and detailed documentation of all patient interactions and interventions

n

Participating in ongoing professional development to stay up-to-date with best practices in social work

n

What we're looking for

n

Bachelor's or master's degree in Social Work or a related field

n

Current registration or eligibility for registration with the Australian Association of Social Workers (AASW)

n

Minimum of 2 years of experience working as a Social Worker,



preferably in a healthcare or community-based setting

n

Strong interpersonal and communication skills, with the ability to empathize and build trust with patients and their families

n

Excellent problem-solving and critical thinking skills, with the ability to navigate complex situations and make informed decisions

n

Commitment to upholding ethical standards and maintaining confidentiality

n

Proficiency in using relevant software and technology to support patient care and documentation
