Job Description

HFH CARE PTY LTD is seeking experienced and reliable Support Workers to provide ongoing support to a male participant in his 60s with complex, high-support needs. The participant has a brain injury, cerebral palsy, and one amputated leg. He is able to walk with assistance and also uses a wheelchair. He lives at home with his wife and requires support with personal care, mobility, community access, meal preparation, toileting, and overnight assistance.

Key responsibilities

- Provide showering and full morning personal care
n
- Assist with dressing and hygiene support
n
- Prepare breakfast and dinner
n
- Provide toileting and continence assistance
n
- Assist with mobility and transfers
n
- Support community access and transport to medical and other appointments
n
- Provide assistance with daily activities around the home
n
- Provide active overnight support and remain awake and available throughout night shifts to respond to the participant's needs, including assistance with transfers and toileting approximately 4-5 times throughout the night

- Provide general companionship and support

About you

n

n
- Previous experience supporting participants with complex or high physical support needs
n
- Experience with personal care, continence support, transfers, mobility assistance, and wheelchair support
n
- A nursing, disability, aged care, or similar healthcare background is highly regarded
n
- Solid spoken English and excellent communication skills
n
- Confidence providing respectful and dignified personal care
n
- Reliability, patience, professionalism, and a calm approach
n
- Ability to work independently and follow established care routines
n
- Availability for regular and ongoing shifts
n
- All relevant checks, qualifications, and requirements applicable to disability support work, including appropriate screening, First Aid/CPR, and a valid driver licence where transport duties are required
