Job Description

Premier Supported Living Group is seeking an experienced Support Coordinator / Case Manager to join our Melbourne team. You will coordinate supports for NDIS participants, including hospital transitions, SIL, and community services, liaising with families, clinicians and service providers to achieve meaningful outcomes.

The role requires solid organisation, communication and problem-solving skills, with a participant-centred approach to planning and reviewing supports in line with NDIS Practice
