Connections Home Care & Nursing is looking for experienced disability support workers who have experience supporting clients living with various disabilities.

n

Immediate start, training and development opportunities.

The successful candidate must:

n

- have a wide availability- Monday-Sunday
n
- be able to support clients throughout Melbourne
n
- have a current valid Victorian driver's license and access to a car to transport clients (desirable)
n
- Non-drivers are also welcome to apply

Key Responsibilities

n

The support required for our clients include:

n

n
- Assisting clients with daily living activities such as bathing, dressing, grooming, toileting, and meal preparation while promoting dignity and independence
n
- Encouraging and assisting clients to participate in social, recreational, educational, and community activities to enhance inclusion and quality of life.
n
- Helping clients develop and maintain skills such as cooking, household cleaning, budgeting, shopping, and using public transport to increase independence.
n
- Observing and reporting changes in a client's physical, emotional, or mental health, administering medication (where qualified and authorised), and following care plans.
n




- Keeping detailed documentation of services provided, incidents, and client progress while ensuring compliance with workplace policies, safety procedures, and individual support plans
n

Skill & Experience

n

Preferred but not essential:

n

- Certificate III or IV Individual Support
n
- Certificate IV Disability
n
- Certificate of IV Aged Care
n
- Nursing
n
