Anglicare Victoria South Division, Keep Embracing Your Success (KEYS) residential program are looking for a for full time (SCHADS 6) Senior Case Manager to join our team based in Dandenong.

About the KEYS Program

n

In KEYS, Anglicare Victoria, in collaboration with DFFH Child Protection, MIND Australia, Monash Health, VACCA, and the Youth Support and Advocacy Service (YSAS), supports young people aged 12–17 years experiencing complex emotional and behavioural challenges.

n

The program operates across two residential homes in Dandenong and Noble Park, each accommodating four young people. The Senior Case Manager will manage up to eight cases:

- Four young people in residential care (Phase 1).
n
- Four young people transitioned to community-based living (Phase 2).
n

Key Responsibilities

n

Provide intensive case management and direct support to young people in KEYS.

n

Coordinate care teams, ensuring optimal supports and effective transition planning.

n

Collaborate with Monash Health, YSAS, VACCA, Mind Australia, and DFFH.

n

Manage program administration, including case notes, reports, and KEYS assessment plans

n
