About the Role

n

We're looking for an experienced Service Coordinator with a strong background in NDIS service delivery and case management.

n

You'll coordinate services for a medium-sized, manageable group of participants, working closely with families, representatives, support coordinators, support workers, and external providers to deliver person-centred, high-quality supports.

This non-clinical role focuses on coordinating day-to-day services, maintaining solid stakeholder relationships, supporting frontline staff, and ensuring services meet participant goals and NDIS requirements.

Key Responsibilities

n

n
- Build and maintain productive relationships with participants, families, representatives, support coordinators, and external providers
n
- Coordinate participant services from onboarding through to ongoing review and service transition
n
- Develop, review, and update support plans, service agreements, risk assessments, and progress reports
n
- Ensure supports align with participant goals, preferences, funding, and NDIS plans
n
- Monitor service quality, participant satisfaction, goal progress, and changing support needs
n
- Conduct regular participant reviews, stakeholder meetings, and service check-ins
n




- Work closely with rostering and operational teams to ensure consistent service delivery
n
- Provide guidance and day-to-day support to disability support workers
n
- Respond to participant concerns, incidents, service issues, and changes in circumstances
n
- Maintain accurate case notes, participant records, and compliance documentation
n
- Support incident investigations, corrective actions, and required reporting
n
- Ensure services comply with NDIS Practice Standards and organisational policies
n
- Identify risks, service gaps, and opportunities for continuous improvement
n

Your Experience and Qualifications

n

n
- Strong service coordination and case management experience within the NDIS, disability, or community services sector
n
- Demonstrated understanding of NDIS participant plans, funded supports, service agreements, and provider responsibilities
n
- Sound knowledge of NDIS Practice Standards and Quality Indicators
n
- Understanding of NDIS Commission requirements, including incident management, reportable incidents, complaints, and safeguarding
n
- Experience coordinating supports for people with complex needs, preferably within SIL, accommodation, or community settings
n
- Proven ability to coordinate multiple stakeholders and manage differing priorities
n
- Experience developing participant support plans, risk assessments, and service documentation
n
- Strong communication, organisation, problem-solving, and conflict-resolution skills
n
- Ability to work independently while contributing effectively to a team
n
- Experience supervising or supporting disability support workers
n
- Relevant qualification in disability, community services, case management, or a related discipline is desirable
n
- Valid SA driver's licence, First Aid certificate, NDIS Worker Screening Check, and required clearances
n
