Ramahyuck District Aboriginal Corporation (RDAC) was established as an Aboriginal Community Controlled organisation in 1992 to improve the social, health and economic outcomes for Aboriginal people. RDAC is a leading provider of primary health care and related social and family support services to Aboriginal people and communities across Gippsland. RDAC's services are provided at the following locations: Wellington and Latrobe local government areas. RDAC's services include: general practice and chronic disease management, allied health, patient transport, drug and alcohol counselling, child and family services, including; Maternal Services and Early Learning Centre, home and community care, and social and emotional wellbeing support and counselling.

The Primary Health Care Team aim to provide holistic quality health care services, which meet the health care needs of the communities in which we deliver services. All Ramahyuck clinic's are accredited against the RACGP Standards and registered with QPA. Our health team professionals provide a comprehensive range of health care services inclusive of: health checks, general medical complaints, immunisation, diabetes education, venepuncture, maternity care and chronic disease management.

About the role

n

This is a Maternity Leave Position.

n

This position will play a significant role in assisting the improvement of health outcomes for patients of Ramahyuck District Aboriginal Corporation and Aboriginal community members, through provision of:

n

- Primary health care
n
- Health education
n
- Early detection of disease
n
- Effective management of disease
n

n

Service Delivery

n

Provide clinical nursing services to clients attending the clinics including but not limited to:

n

n
- Venepuncture
n
- Electrocardiogram and Holter Monitor
n




- Understanding of Victorian Immunisation Schedule, the National Immunisation Program eligibility, funding and administration of immunisations.
n

n

Implementation and support of GPs in preventative health and disease management through health assessments including but not limited to;

n

n
- Aboriginal and Torres Strait Islander Health Check
n
- GP Chronic Condition Management Plan
n
- Heart Health Check
n

n

Understanding of health screening including but not limited to;

n

n
- Cancer (Cervical, Bowel, Lung, Breast)
n
- Metabolic Health
n
- General patient observations
n
- Health promotion
n

n

Knowledge and maintenance of recall and reminder system.

n

Provide a link between services and patients, through networking and liaising with:

n

n
- Local hospitals and GP clinics
n
- Home Visiting Services
n
- Aged Care Assessment Teams
n
- Allied Health Providers
n
- GPs / Specialists
n
- NDIS Service Providers
n
- Services Australia
n
- Department of Health
n

Management of the clinical setting by assisting the service to meet relevant standards and legislative requirements in:

- Oversite of stock control for medical supplies and immunisations
n
- Oversite of signed monthly clinical checks
n

Position Requirements

- Registered Nurse - Division 1 with the Australian Health Practitioner Regulation Agency
n
- Current Working with Children's Check
n
- Satisfactory Criminal History Check
n
- Current Victorian Driver's License
n
- Satisfactory pre-employment pass drug & alcohol screening
