Townsville Hospital and Health Service is seeking a Discharge Coordinator for the Adult Acute Mental Health Inpatient Unit (AAMHIU) at Townsville University Hospital. You will coordinate and facilitate discharges, work with the CNC and Shift Coordinator, and support families and carers to ensure secure, timely discharge and optimal use of resources.
You will participate in reviews of consumers and ensure warm handovers are completed with all relevant documentation to support continuity of care.
