Working within the Port Lincoln Community & Allied Health unit, you'll deliver person-centred nursing care to individuals and families across the lifespan in community-based settings.

Key responsibilities

- Conducting health assessments and delivering evidence-based nursing care within the community.
n
- Coordinating care plans and working collaboratively with allied health, medical, and community support teams.
n
- Supporting chronic disease management, health education, and early intervention programs.
n
- Maintaining accurate clinical documentation in line with SA Health standards and legislative requirements.

What you need
