About the role

n

We are seeking a Social & Emotional Wellbeing Care Coordinator to provide coordination of supports to Aboriginal and Torres Strait Islander clients who are experiencing mental health and/or substance misuse conditions. This role has a focus of kids in out of home care, and typically works with people aged 0-19 years. This full time fixed term (12 months - parental leave) position is available at our Coomera clinic, although applicants seeking part time (e.g. 0.8 FTE) will be considered.

What you'll be doing

n

n
- Triage & assessment of incoming referrals
n
- Maintain the intake register and manage wait lists effectively and timely
n
- Collaborate with the Senior Practitioner to assist with triage of referrals
n
- Undertake assessment needs of clients and effectively manage and coordinate service delivery and provide case management
n




- Provide brief intervention for clients presenting for mental health wellbeing and substance misuse
n
- Respond to crisis/acute presentations
n
- Provide access and coordination to comprehensive support services for clients with the aim of achieving wellbeing as part of a holistic model of care
n
- Support the goals of the client and that the goals are focused on building the resilience and capacity of the client
n
- Work with, & provide guidance for support workers to coordinate & co-manage care for clients referred to the program

- Builds strong, respectful relationships with Aboriginal and Torres Strait Islander communities
n
- Brings experience in work with diverse people and presentations in providing case managed social work driven supports
n
- Is passionate about making a positive impact and supporting others with heart and purpose
n

- Bachelor or Diploma in Health, Human Services, Community Services sector or relevant field is highly desirable
n

Requirements

- A Blue Card for working with children ('No Card No Start' policy applies)
n
- National police check
n
