The District Nurse (DN) will be required to visit the homes of the community clients requiring support withmedication management, post-acute care, chronic disease management including respiratory illness, cardiac anddementia support and maintenance, wound care, palliative care, and frail and aged support. DN is responsiblefor the total direct / indirect management and education of the client, and family involvement in direct / indirectnursing care.

The District Nurse works closely with the MT@Home team to deliver coordinated, person-centred care,supporting clients' health, wellbeing and ability to remain connected to their community. MT@Home providesin-home support services to older people, enabling them to maintain their independence and continue livingsafely in their own homes for as long as possible.

Selection criteria

n

n
- Proven ability to apply high ethical standards, and represent MTHCS in a qualified manner.
n
- Registered Nurse qualification and current registration with AHPRA.
n
- Demonstrated knowledge and understanding of clinical nursing pathways.
n
- Effective interpersonal and communication skills applicable to a broad range of clients.
n
- Knowledge of and commitment to the principles of primary health and health promotion.
n
- Demonstrated skills and experience in the delivery of high-quality healthcare services in a rural community setting.
n
- Ability to provide high-quality wound care, including assessment, treatment planning and monitoring of healing outcomes.
n
- High-level communication skills and the ability to work autonomously within a multidisciplinary team.
n




- Current Driver's Licence.
n
- Current Police Check.
n
- Experience in rural health.
n
- Other specialist clinical qualifications relevant to the District Nursing role.
n
- Experience in community work, including well-developed facilitation, problem-solving, negotiation and resolution skills.
n

Key responsibilities

n

n
- Deliver person-centred nursing care, including assessment, planning, implementation, evaluation and review of care in collaboration with clients, their families and representatives, and the multidisciplinary healthcare team, including MT@Home.
n
- Coordinate service delivery, referral management and caseload activities as part of the District Nursing Team.
n
- Maintain accountability for the quality, safety and continuity of care.
n
- Liaise effectively with nursing, medical, allied health and community service providers to achieve positive client outcomes.
n
- Ensure services are delivered in accordance with MTHCS policies, professional standards, legislation, funding program requirements and contemporary nursing practice.
n
- Contribute to the development, implementation and continuous improvement of clinical practices, policies, procedures and service initiatives.
n
- Maintain accurate, timely and confidential clinical documentation, statistics and reporting requirements, and ensure documentation platforms are updated correctly.
n
- Participate in mandatory training, professional development, staff meetings, case conferences and quality improvement activities.
n
- Foster a protected, respectful and supportive environment for clients, visitors and staff.
n
- Promote client independence, wellbeing and health literacy through education, support and health promotion activities where needed.

- Demonstrate professionalism, effective communication, problem‐solving skills and positive representation of MTHCS at all times.
n
- Undertake other duties within the scope of the role as directed.

Section 21 of the Occupational Health & Safety Act 2004, requires an Employer to provide and maintain, so far as is reasonable and practicable, a safe workplace for all employees and ensure that no employee is placed at risk of injury while performingthe duties and responsibilities required by their position.

If a risk is identified and fitness to safely perform the duties of the position needs to be determined, MTHCS with the agreement of the candidate/employee, will arrange an independent medical assessment to determine capacity to safely perform the inherentrequirements of the position to minimise the risk of injury or aggravation of any pre-existing condition the candidate/employee may have.
