Our Hospital in the Home (HITH) service enables patients who would otherwise require hospital admission to safely receive acute-equivalent treatment in their homes, workplaces, residential aged care facilities, or through virtual care pathways.

About the Opportunity

n

n

We're seeking experienced Clinical Nurses to join our growing Hospital in the Home (HITH) team based at St Vincent's Private Hospital Sydney.

n

As a HITH Clinical Nurse, you'll deliver high-quality, evidence-based care outside traditional hospital walls, supporting patients with acute and complex healthcare needs in their preferred environment.

n

Working closely with Clinical Leads, Visiting Medical Officers, allied health professionals and community providers, you'll play a critical role in ensuring patients receive safe,



coordinated and compassionate care while maintaining hospital-level clinical standards

n

Key Responsibilities

n

n

n
- n

Conduct comprehensive patient assessments and clinical reviews.

n
- n

Develop, implement and evaluate individualised care plans.

n
- n

Deliver advanced nursing interventions, including IV therapies and complex wound management.

n
- n

Monitor patients with acute and complex clinical needs and escalate care when appropriate.

n
- n

Coordinate referrals, admissions, transitions of care and discharge planning.

n
- n

Collaborate with multidisciplinary teams to ensure seamless care delivery.

n
- n

Educate and support patients, families and carers.

n
- n

Participate in service development, quality improvement and clinical governance activities.

n
- n

Mentor and support new and junior team members.

n

n

About You

n

n

You're an experienced Registered Nurse who enjoys building meaningful patient relationships and thrives in an environment that combines autonomy, innovation and clinical excellence.You are passionate about delivering high-quality care in the community and are excited by the opportunity to help shape the future of connected healthcare.

You Will Bring

n

• Current registration as a Registered Nurse with AHPRA.
n • Minimum three (3) years' post-graduate nursing experience.
n • Experience in community nursing and/or acute medical, surgical, emergency, ICU or critical care environments.
n •Demonstrated experience managing patients with complex clinical needs and multiple co-morbidities.
n • Strong clinical assessment, care planning and problem-solving skills.
n • Excellent communication and stakeholder engagement capabilities.
n •Ability to work independently while contributing effectively within a multidisciplinary team

n

• Solid digital literacy and confidence using virtual care technologies.
n • Current unrestricted NSW Driver's Licence.
n • Full and unrestricted working rights in Australia.

n

Highly Regarded

n

• Hospital in the Home (HITH), community nursing, virtual care or care-at-home experience.
n • Postgraduate qualifications in nursing or related specialty areas.
n • Experience contributing to service innovation, quality improvement or clinical projects.
