Physiotherapist - Community Aged Care - West Sydney, NSW

About the Role: We are seeking an experienced Aged Care Physiotherapist to join an expanding aged care and disability service provider for a community-based role in Sydney's West, NSW. Full-time and part-time arrangements are available. In this community-based role, you will deliver clinical care to clients funded through Home Care Packages, the Commonwealth Home Support Programme, and the NDIS. You will provide client-focused physiotherapy services and conduct comprehensive assessments across a diverse caseload. Using evidence-based practice, you will design and implement tailored rehabilitation programs, ensuring your treatment plans are well-justified and outcome-driven. You will also collaborate closely with fellow allied health professionals, care coordinators and facility managers, while contributing to ongoing education and professional development initiatives.

About You:

- Relevant qualification in Physiotherapy
- Current AHPRA registration
- Demonstrated experience in aged care and community physiotherapy (essential)
- A passion for high-quality service delivery
- Excellent communication and report writing skills
- Strong organisational skills and an ability to work autonomously
- Highly organised and efficient
- Current driver's licence
- Police Check
- Eligible to work permanently in Australia
